Submitting off-site forms
How do I submit completed forms?
Step 1: Review forms for accuracy before submitting them for approval
Step 2: Submit forms to your administrator for review and approval
Simple trips are approved by your school administrator
Complex trips only
Step 3: Signed complex forms are submitted to superintendents or executive
directors for final approval
Forms will be forwarded as required for final approvals:
Out-of-territory trips are approved by the deputy minister
International trips are approved by the minister
Off-site trip documentation must be handled in accordance with the Archives Act.
All completed forms must be kept on file at the school office.
Complex trip risk analysis and contingency plans must be specific to the trip, location, activity and season
Applications will be returned for editing if they appear incomplete or inaccurate

When do I have to submit complex forms?
Complex trips: 2 weeks before trip (3 weeks is better!)
Out-of-territory trips: 4 weeks before the trip (6 weeks is better!)
Complex trip paperwork from Yukon Education schools is reviewed by the
Off-Site Review Committee (OSRC), who meet every two weeks.
